Looking at a particular Agile Development scenario - With tight delivery schedules, resources working for extended hours, numerous test runs of the release, fixing of bugs and again conducting the regression, I think we are really missing out on Quality. Where is the Configuration of the artifacts being taken care of, where are the lessons learnt being captured, are we capturing any metrics or are we just fixing the bugs without analyzing their trend and root cause, where is the safety of the systems being delivered getting taken care of.

With Agile development now being a part of Safety Critical Systems as well, it is the time we really need to understand importance of Quality and Processes in Agile Development before we miss out on something BIG.
